The replacement CLSIS forms are available for download here. The table outlines the forms that have been updated.
The changes are primarily focused on fitting in the new data items needed per CLASS/Data Standards Manual. Changes include addition of financial disadvantage indicator and additional income brackets. A centre might use these as a guide to update their own forms, or customise these for their own practice, particularly if they prefer a different form for every service type.
